Life Insurance Policy benefit in the amount of $5,000 in provided at no cost to all permanent, full-time employees working a minimum of 30 hours per week. Eligibility begins the 1st of the next month after 90 days consecutive employment. Additional benefits may be available if policyholder opted to purchase additional coverage
