ONE-TIME DEATH BENEFIT

Firefighters who are actively employed in a position included in the Broward County Personnel Cap and regularly working at least 20 hours per week. Eligibility for benefit begins the first day of the calendar month coinciding with or next following 60 consecutive days of active employment.

Life Insurance
Policy benefit in the amount $25,000 is provided by the county at no cost to employees. Additional coverage may apply if employee contributed toward additional life insurance.

Repatriation Benefit
Coverage of expenses incurred to transport body to a mortuary near primary place of residence, but not to exceed $5,000 of 10% of Life Insurance Benefit, whichever is less.

Accidental Death & Dismemberment Insurance
Amount is equal to the amount of employee’s Life Insurance Benefit. Applies when death is caused solely and directly by an accident; occurs independently of all other causes; and occurs within 365 days after the accident.

Seat Belt Benefit
The amount of the Seat Belt Benefit is the lesser of $10,000 or the amount of AD&D Insurance Benefit payable for loss of life. It applies when death was the result of an automobile accident for which an AD&D Insurance Benefit is payable for Loss of Life and policyholder was wearing and properly utilizing a Seat Belt System at the time of the accident, as evidenced by a police accident report.

Firefighters who are actively employed in a position included in the Broward County Personnel Cap and regularly working at least 20 hours per week. Eligibility for benefit begins the first day of the calendar month coinciding with or next following 60 consecutive days of active employment.

Child Care Benefit
Total child care expense incurred by surviving spouse or domestic partner within 36 months after date of death for all children under age 13, but not to exceed $5,000 per year or the cumulative total of $10,000 or 25% of the AD&D Insurance Benefit, whichever is less. Applies if a) policyholder is insured under Group Policy; b) death is the result of an accident for which an AD&D Insurance Benefit is payable for Loss of Life; c) Spouse or Domestic Partner pays a licensed child care provider who is not a member of your family for child care provided to child(ren) under age 13 within 36 months of death; and d) child care is necessary in order for Spouse or Domestic Partner to work or to obtain training for work or to increase earnings. In situations where there is no surviving Spouse or Domestic Partner, no Child Care Benefit will be paid.

Higher Education Benefit
Tuition expenses incurred per child within 48 months after date of death at an accredited institution of higher education, exclusive of board and room, books, fees, supplies and other expenses, but not to exceed $5,000 per year, or the cumulative total of $20,000 or 25% of the AD&D Insurance Benefit, whichever is less. Applies if a) policyholder is insured under Group Policy; b) death is the result of an accident for which an AD&D Insurance Benefit is payable for Loss of Life; and c) child(ren) is(are), within 12 months after date of death, registered and in full-time attendance at an accredited institution of higher education beyond high school.

Career Adjustment Benefit
Tuition expenses for training incurred by Spouse or Domestic Partner within 36 months after the date of death, exclusive of board and room, books, fees, supplies, and other expenses, but not to exceed $5,000 per year, or the cumulative total of $10,000 or 25% of the AD&D Insurance Benefit, whichever is less. Applies if a) policyholder is insured under Group Policy; b) death is the result of an accident for which an AD&D Insurance Benefit is payable for Loss of Life; and c) Spouse or Domestic Partner, within 36 months after date of death, registers and is in attendance at a professional or trades training program for the purpose of obtaining employment or increasing earnings.

Provides $5,000 lump sum assistance with immediate needs. Later will review overall financial status of family. Provides scholarships to spouse and children at the University of Florida at Gainesville and Florida State University at Tallahassee.
